ABSTRACT

Objetivo: verificar a demanda de hemoculturas, aspirados traqueais e uroculturas realizadas no HU-UNIVASF/ EBSERH e a prevalência dos microrganismos identificados no período de janeiro a junho de 2016. Métodos: estudo retrospectivo documental com abordagem quantitativa. Resultados: o setor de microbiologia realizou 488 hemoculturas, 427 uroculturas e 197 aspirados traqueais. A positividade de hemoculturas mostrou-se entre 10,9 à 25,7%, e o percentual de contaminações variou de 6,8 à 14,0%. Os microrganismos mais prevalência nas hemoculturas foram Staphylococcus epidermidis (23,7%), Staphylococcus aureus (19,3%) e Klebisiella pneumoniae (9,6%). Nas uroculturas foram Klebisiella pneumoniae (23,1%), Candida sp. (13,5%) e Escherichia coli (12,5%). Nos aspirados traqueais foram Acinetobacter baumannii (29,2%), Pseudomonas aeruginosa (26,6%) e Staphylococcus aureus (16,2%). Conclusão: a cultura mais solicitada foi hemocultura. A bactéria mais prevalente nas hemoculturas foi Staphylococcus epidermidis, nos aspirados traqueais Acinetobacter baumannii e nas uroculturas Klebisiella pneumoniae


Objective: the study's purpose has been to verify the demand for blood cultures, tracheal aspirates and urine cultures performed at a University Hospital from the Universidade Federal do Vale do São Francisco (HU-UNIVASF/EBSERH), as well as the predominance of microorganisms identified over the period from January to June 2016. Methods: it is a retrospective documentary study with a quantitative approach. Results: the microbiology sector carried out 488 blood cultures, 427 urine cultures and 197 tracheal aspirates. The positivity of blood cultures was between 10.9 and 25.7%, and the percentage of contaminations ranged from 6.8 to 14.0%. The most prevalent microorganisms in blood cultures were Staphylococcus epidermidis (23.7%), Staphylococcus aureus (19.3%) and Klebsiella pneumoniae (9.6%). In urine cultures were Klebsiella pneumoniae (23.1%), Candida sp. (13.5%) and Escherichia coli (12.5%). In tracheal aspirates were Acinetobacter baumannii (29.2%), Pseudomonas aeruginosa (26.6%) and Staphylococcus aureus (16.2%). Conclusion: the most requested culture was blood culture. The most prevalent bacterium in blood cultures was Staphylococcus epidermidis, in tracheal aspirates was Acinetobacter baumannii, and in urine cultures was Klebsiella pneumoniae


Objetivo: el propósito del trabajo es verificar la demanda de hemocultivos, aspirados traqueales y urocultivos realizados en el Hospital Universitário de la Universidade Federal do Vale do São Francisco (HU-UNIVASF/ EBSERH) y la prevalencia de los microorganismos identificados en el período de enero a junio de 2016. Métodos: este trabajo es un estudio retrospectivo documental con abordaje cuantitativo. Resultados: el sector de microbiología realizó 488 hemocultivos, 427 urocultivos y 197 aspirados traqueales. La positividad de hemocultivos se mostró entre el 10,9 al 25,7%, y el porcentaje de contaminaciones varía de 6,8 a 14,0%. Los microorganismos más prevalentes en los hemocultivos fueron Staphylococcus epidermidis (23,7%), Staphylococcus aureus (19,3%) y Klebsiella pneumoniae (9,6%). ABSTRACT

ABSTRACT A 65-year-old male with a history of urinary tract trauma requiring cystotomy and chronic bladder catheterization, presenting with chronic and uninvestigated changes in the color of the urine bag system, with no urine color change, and positive urine culture for Proteus mirabilis . These characteristics refer to the purple urine bag syndrome, a not weel-known condition, with a benign course in most cases, and associated with urinary tract infection in patients with chronic bladder catheterization. Although it is characterized by marked changes, it is underdiagnosed by healthcare professionals.


RESUMO Homem de 65 anos com história de trauma do sistema urinário, sendo necessário cistotomia e sondagem vesical crônica, apresentando alterações crônicas e não investigadas da cor do sistema coletor de urina, sem alteração da cor da urina, e urocultura positiva para Proteus mirabilis . Tais características remetem à síndrome do saco coletor de urina roxo, uma entidade pouco conhecida, de curso benigno na maioria da vezes, associada à infecção urinária em paciente com cateterismo vesical de demora. Embora seja caracterizada por alterações marcantes, é subdiagnosticada pelos profissionais de saúde.

Resumen Introducción. Los caballos de trabajo de la Policía Nacional tienen un estrecho contacto con sus manejadores y la población en general durante las actividades recreativas y de patrullaje, lo cual puede favorecer la transmisión de la leptospirosis en los caballos y el personal ocupacionalmente expuesto. Objetivo. Caracterizar epidemiológicamente la leptospirosis mediante pruebas de serología, urocultivo y reacción en cadena de la polimerasa (Polymerase Chain Reaction, PCR) en caballos de trabajo y personal con riesgo ocupacional pertenecientes a seis unidades de la Policía Nacional de Colombia. Materiales y métodos. Se evaluaron 153 caballos machos castrados y 123 personas en las seis unidades en los municipios de Manizales, Pereira, Armenia, Ibagué, Tuluá y Cali. Se utilizaron tres formatos estructurados para recabar información y se obtuvieron muestras sanguíneas de las personas y de los caballos, las cuales se procesaron con la prueba de aglutinación microscópica (Macroscopic Agglutination Test, MAT) para 24 serogrupos. Se practicó el examen clínico de los caballos y se obtuvieron muestras de orina para el urocultivo y la PCR convencional. Resultados. La seroprevalencia de Leptospira spp. fue de 3,25 % (n=4) en las personas y de 85 % (n=130) en los caballos. Entre los caballos, los serogrupos Djasiman y Shermani fueron los más prevalentes. El urocultivo fue positivo en el 64,7 % (99/153) de las muestras, en tanto que los análisis de PCR fueron negativos. Se encontró una asociación estadísticamente significativa de la frecuencia de salida de las instalaciones (p=0,009) y la presencia de fauna silvestre (p=0,051) con la infección por el serogrupo Shermani. Conclusión. Las características epidemiológicas de la leptospirosis en los caballos sugieren una presentación endémica de la infección y su papel como reservorios de la bacteria; sin embargo, debe dilucidarse la patogenia de la enfermedad con estudios complementarios.


Abstract Introduction: Police working horses are in close contact with their managers and the general population during recreational and patrol activities, which can favor the transmission of leptospirosis among the horses and the occupationally exposed personnel. Objective. To characterize epidemiologically leptospirosis through serology, urine culture and PCR in working horses and in the occupationally exposed population in six police stations in Colombia. Materials and methods. We tested 153 castrated male horses and 123 people in six police stations in the municipalities of Manizales, Pereira, Armenia, Ibagué, Tuluá, and Cali. Three structured formats were applied and blood samples were obtained from people and horses, which were processed with the Macroscopic Agglutination Test, (MAT) for 24 serogroups. Horses were subject to a clinical examination, and urine samples were obtained for urine culture and conventional PCR. Results. The seroprevalence of human Leptospira spp. was 3.25% (n=4) while in horses it was 85% (n=130). Among the horses, serogroups Djasiman and Shermani were the most prevalent. The urine culture was positive in 64.7% (99/153) of the samples, whereas PCR analyzes were negative. A statistically significant association was found between the frequency of exiting the facilities (p=0.009) and the presence of wildlife (p=0.0051) with the infection by serogroup Shermani. Conclusion. The epidemiological characteristics of leptospirosis in horses suggest an endemic presentation of the infection and its role as reservoirs of the bacteria; however, it is necessary to elucidate the pathogenesis of the disease with complementary studies.

RESUMEN OBJETIVO: Ureaplasma urealyticum es el agente más frecuentemente aislado en infección intraamniótica. Los macrólidos son los antimicrobianos de primera elección en embarazadas. Se ha descrito el aumento de resistencia, pudiendo limitar las opciones terapéuticas durante la gestación. El propósito del estudio es evaluar susceptibilidad antimicrobiana de Ureaplasma urealyticum aislado en mujeres en edad fértil, que se atienden en Clínica Alemana Temuco, Araucanía, Chile. METODO: Se estudian todas las muestras de orina y flujo vaginal para cultivo de U. urealyticum, de pacientes entre 18 y 40 años, recibidas en el Laboratorio de Microbiología Clínica Alemana Temuco, en período Abril 2013 a Enero 2015. Se procesan las muestras con kit Mycoplasma IST 2 de Biomerieux. En las que resultan positivas, se estudia susceptibilidad a macrólidos, tetraciclinas y quinolonas. RESULTADOS: 426 muestras de orina y flujo vaginal (390 pacientes). 197 pacientes resultaron positivas para U. urealyticum. (50,5%). La susceptibilidad fue 88,4% (174 pctes) a Eritromicina, 87,9% (173 pctes) a Claritromicina y 91,9% (181 pctes) a Azitromicina (NS). 15 de 197 pacientes (7,6%) fueron resistentes a los 3 macrólidos. La susceptibilidad a Quinolonas fue 55,3% a Ciprofloxacino, y 94% a Ofloxacino. El 100% resultó susceptible a Tetraciclinas. CONCLUSIONES: Cerca del 10% de U. urealyticum aislados en nuestra serie son resistentes a macrólidos, contribuyendo a la no erradicación de la infección en tratamientos empíricos. Dentro de ellos, azitromicina aparece con la mayor efectividad. El aumento de resistencia limitará opciones terapéuticas, con gran impacto perinatal en futuro. La vigilancia de susceptibilidad en cada hospital es fundamental para elección terapéutica.


ABSTRACT INTRODUCTION: Ureaplasma urealyticum is the most frequently isolated microorganism in intra-amniotic infection. The macrolides are the first choice antimicrobials for treat this infection in pregnancy. The increasing resistance has been described worldwide, seriously limiting therapeutic options in pregnancy. The aim of the study is to evaluate antimicrobial susceptibility of U. urealyticum aislated in fertile-age women in Clínica Alemana Temuco, Araucania region, Chile. METHOD: Urine and vaginal samples were analyzed for U. urealyticum, from every 18 to 40 years old patients, received at Microbiology Laboratory of Clínica Alemana Temuco, between April 2013 to January 2015. The samples are processed with Mycoplasma IST 2 kit of Biomerieux. If they became positives, susceptibility to macrolides, tetracyclines and quinolones was studied. RESULTS: 426 urine and vaginal samples were collected (390 patients). 197 patients were positive for U. urealyticum (50.5%). The susceptibility was 88.4% (174 pts) to Erythromicyn, 87.9% (173 pts) to Clarithromycin and 91.9% (181 pts) to Azithromycin (NS). Resistance to all macrolides was observed in 15 out of 197 patients (7.6%). The susceptibility to Quinolones was 55.3% to Ciprofloxacin, and 94% to Ofloxacin. The 100% was susceptible to Tetracyclines. DISCUSSION: Near to 10% of isolated Ureaplasma spp in our serie were resistant to some macrolide, being a factor for failing to eradicate the infection in empirical treatment. Azithromycin was the most effective. The increasing resistance will limit therapeutic options, with great perinatal impact in the future. Susceptibility surveillance in each hospital is very important for therapeutic options.

ABSTRACT Background: Formation of struvite stones is associated with urinary tract infection by urease-producing bacteria. Biogenic crystal growth in natural and synthetic materials is regulated by the action of inhibitors, ranging from small ions, molecules to large macromolecules. Materials and Methods: We report the dynamics of in vitro crystallization of struvite in presence of vitamin C in synthetic urine using single diffusion gel growth technique. Sodium metasilicate gel of specific gravity 1.05 and the aqueous solution of ammonium dihydrogen phosphate were used as the medium for growing the struvite crystals. The crystallization process was induced by a urease positive struvite stone associated Pseudomonas aeruginosa to mimic the infection leading to stone formation. The grown crystals were characterized by ATR-FTIR and powder XRD. The surface morphology was analysed through FE-SEM for comparison between treatments. Results: We observed decrease in number, dimension, and growth rate of struvite crystals with the increasing concentrations of vitamin C. Crystals displayed well-defined faces and dendritic morphology of struvite in both control and biogenic systems. Conclusion: The results strongly suggest that, vitamin C can modulate the formation of struvite crystals in the presence of uropathogenic bacteria.

ABSTRACT

Abstract A modified TaqMan real-time polymerase chain reaction targeting a 138 bp fragment within the lipl32 gene was developed to identify exclusively pathogenic Leptospira spp. in dog urine samples. Thirty-five samples from dogs with suspected clinical leptospirosis and 116 samples from apparently healthy dogs were tested for presence of leptospiral DNA using the TaqMan-based assay. The results were compared with those from a well-established conventional PCR targeting the 16S RNA encoding gene associated with nucleotide sequencing analysis. The overall agreement between the assays was 94.8% (confidence interval [CI] 95% 88-100%). The newly developed assay presented 91.6% (CI 95% 71.5-98.5%) relative sensitivity (22[+] lipl32 PCR/24[+] 16S RNA and sequencing), 100% (CI 95% 96.3-100%) relative specificity and 98.7% accuracy (CI 95% 94.8-100%). The lipl32 assay was able to detect and quantify at least 10 genome equivalents/reaction. DNA extracted from 17 pathogenic Leptospira spp., 8 intermediate/saprophytic strains and 21 different pathogenic microorganisms were also tested using the lipl32 assay, resulting in amplification exclusively for pathogenic leptospiral strains. The results also demonstrated high intra and inter-assay reproducibility (coefficient of variation 1.50 and 1.12, respectively), thereby qualifying the newly developed assay as a highly sensitive, specific and reliable diagnostic tool for leptospiral infection in dogs using urine specimens.

ABSTRACT

RESUMEN El tratamiento empírico para la infección urinaria se ve complicado frente a la presencia de multirresistencia y de betalactamasas de espectro extendido (BLEE). El objetivo del estudio fue describir los patrones de resistencia antibiótica de cepas de Escherichia coli aisladas en urocultivos y los factores clínico-epidemiológicos asociados a la presencia de BLEE en un grupo pediátrico y adulto. Se recolectaron durante 14 meses, 353 cepas provenientes de Emergencia y Hospitalización del Hospital Cayetano Heredia, 45,9% fueron cepas multirresistentes. La incidencia de BLEE en población pediátrica fue 16,3% vs. 31,1% en la adulta, el 63,6% provenía de pacientes ambulatorios. La presencia de BLEE se asoció con encontrarse hospitalizado en pediatría, así cómo al uso de pañal y vejiga neurogénica en adultos. Estos factores deben considerarse al momento de elegir un tratamiento antibiótico. Asimismo, es necesario implementar programas de reporte epidemiológico y modelos de prevención de factores de riesgo.


ABSTRACT The empirical treatment of urinary infections is complicated by the presence of multiresistance and resistance to extendedspectrum beta-lactamases (ESBLs). The objective of this study was to describe the patterns of antibiotic resistance of Escherichia coli strains isolated from urine cultures and the clinical-epidemiological factors associated with the presence of ESBLs in a pediatric and an adult group. A total of 353 strains were collected from the Emergency and Hospitalization Sector of the Cayetano Heredia Hospital over 14 months, and 45.9% of the isolated strains were multiresistant. The rate of resistance to ESBLs in the pediatric and adult population was 16.3% and 31.1%, respectively, and 63.6% of the resistant strains were isolated from outpatients. The presence of ESBLs was associated with hospitalization in pediatrics, use of diapers, and the presence of neurogenic bladder in adults. These factors should be considered in selection of an antibiotic treatment. Moreover, epidemiological reporting programs and models should be implemented for reduction of risk factors.

ABSTRACT Objective To investigate whether the urine dipstick screening test can be used to predict urine culture results. Methods A retrospective study conducted between January and December 2014 based on data from 8,587 patients with a medical order for urine dipstick test, urine sediment analysis and urine culture. Sensitivity, specificity, positive and negative predictive values were determined and ROC curve analysis was performed. Results The percentage of positive cultures was 17.5%. Nitrite had 28% sensitivity and 99% specificity, with positive and negative predictive values of 89% and 87%, respectively. Leukocyte esterase had 79% sensitivity and 84% specificity, with positive and negative predictive values of 51% and 95%, respectively. The combination of positive nitrite or positive leukocyte esterase tests had 85% sensitivity and 84% specificity, with positive and negative predictive values of 53% and 96%, respectively. Positive urinary sediment (more than ten leukocytes per microliter) had 92% sensitivity and 71% specificity, with positive and negative predictive values of 40% and 98%, respectively. The combination of nitrite positive test and positive urinary sediment had 82% sensitivity and 99% specificity, with positive and negative predictive values of 91% and 98%, respectively. The combination of nitrite or leukocyte esterase positive tests and positive urinary sediment had the highest sensitivity (94%) and specificity (84%), with positive and negative predictive values of 58% and 99%, respectively. Based on ROC curve analysis, the best indicator of positive urine culture was the combination of positives leukocyte esterase or nitrite tests and positive urinary sediment, followed by positives leukocyte and nitrite tests, positive urinary sediment alone, positive leukocyte esterase test alone, positive nitrite test alone and finally association of positives nitrite and urinary sediment (AUC: 0.845, 0.844, 0.817, 0.814, 0.635 and 0.626, respectively). Conclusion A negative urine culture can be predicted by negative dipstick test results. Therefore, this test may be a reliable predictor of negative urine culture.


RESUMO Objetivo Verificar se a triagem de urina por fitas reativas é capaz de predizer a cultura de urina. Métodos Estudo retrospectivo realizado entre janeiro e dezembro de 2014 com 8.587 pacientes, com solicitação médica de triagem de urina (fita), sedimento urinário e cultura de urina. Foram analisados sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e curva ROC. Resultados Foram positivas 17,5% das culturas. O nitrito apresentou sensibilidade de 28% e especificidade de 99%. O valor preditivo positivo foi de 89% e o valor preditivo negativo de 87%. Esterase apresentou sensibilidade de 79% e especificidade de 84%. Valor preditivo positivo e valor preditivo negativo foram de 51% e 95%, respectivamente. A combinação de nitrito ou esterase positivos apresentou sensibilidade de 85% e especificidade de 84%. Valor preditivo positivo e valor preditivo negativo foram, respectivamente, 53% e 96%. O sedimento positivo (mais de dez leucócitos por microlitro) apresentou sensibilidade de 92% e especificidade de 71%. O valor preditivo positivo foi 40% e o negativo, 98%. A combinação de nitrito e sedimento urinário positivos apresentou sensibilidade de 82% e especificidade de 99%. Os valores preditivos positivo e negativo foram 91% e 98%, respectivamente. Para o nitrito ou esterase positivos mais os leucócitos positivos, a sensibilidade foi de 94% e a especificidade de 84%. O valor preditivo positivo foi de 58% e o negativo foi de 99%. Com base na curva ROC, o melhor indicador de urocultura positiva foi a associação entre a esterase ou nitrito positivos na fita mais os leucócitos positivos no sedimento, seguido por nitrito e esterase positivos, sedimento urinário positivo isolado, esterase positiva isolada, nitrito positivo isolado e, finalmente, pela associação entre nitrito e sedimento urinário positivos (AUC: 0,845, 0,844, 0,817, 0,814, 0,635 e 0,626, respectivamente). Conclusão Uma urocultura negativa pode ser prevista com resultados negativos na fita. Portanto, este teste pode ser um preditor confiável de urocultura negativa.

ABSTRACT

Candida species are the leading cause of invasive fungal infections, and over the past decade there has been an increased isolation of drug resistant Candida species. This study aimed to identify the species distribution of Candida isolates and to determine their unique antifungal susceptibility and resistance patterns. During a cross-sectional study, 209 Candida isolates (recovered from 206 clinical samples) were collected and their species distribution was determined using ChromAgar Candida. The Vitek-2 system (Biomerieux, South Africa) was used to determine minimum inhibitory concentrations (MICs) to azoles (fluconazole, voriconazole), echinocandins (caspofungin, micafungin), polyenes (amphotericin B) and flucytosine. Four species of Candida were isolated, of which C. albicans was the most frequent, isolated in 45.4% (95/209) of the isolates, followed by C. glabrata: 31.1% (65/209). The MICs of the different antifungal drugs varied amongst the species of Candida. From the 130 isolates tested for MICs, 90.77% (112/130) were susceptible to all antifungal drugs and 6.9% (9/130) of the isolates were multi-drug resistant. C. dubliniensis (n=2) isolates were susceptible to all the above mentioned antifungal drugs. There was no significant difference in species distribution amongst clinical specimens and between patients' genders (P>0.05). An increase in MIC values for fluconazole and flucytosine towards the resistance range was observed. To our knowledge, this is the first report on surveillance of Candida species distribution and antifungal susceptibility at a public tertiary teaching hospital in Eastern Cape, South Africa.

Resumen: Objetivo: Describir los patrones de resistencia bacteriana en cultivos de orina de pacientes de un hospital oncológico en la Ciudad de México, de 2004 a 2013. Material y métodos: Se obtuvo el porcentaje de susceptibilidad para diferentes antibióticos, describiendo por separado las bacterias multidrogorresistentes (MDR). Se analizaron por separado las cepas obtenidas de pacientes hospitalizados de las de la comunidad. Resultados: Se realizaron 51 202 cultivos, de los cuales se identificaron 14 480 bacterias (28.3%). De éstas, se reportaron 11 427 Gram negativos (78.9%); 2 080 Gram positivos (14.4%); y 973 (6.6%) levaduras. Escherichia coli fue el principal microorganismo aislado (56.1%); 24% de las cepas de la comunidad y 66% de las nosocomiales fueron productoras de beta-lactamasas de espectro extendido (BLEE). Klebsiella pneumoniae se identificó en 705 cultivos (4.8%), 115 de los cuales fueron BLEE (16%): 13.1% de la comunidad y 29.8% nosocomiales. Pseudomonas aeruginosa se identificó en 593 cultivos (4.1%): 9% de la comunidad y 51% nosocomiales. Conclusiones: Las cepas MDR son mucho más frecuentes en muestras de origen nosocomial. Es prioritario intensificar el uso racional de antibióticos en la comunidad y el programa de desescalamiento de antimicrobianos en el hospital.


Abstract: Objective: To describe the incidence and patterns of bacterial resistance in urine samples from a tertiary care oncology hospital in Mexico, from 2004 to 2013. Materials and methods: We included the strains obtained from urine cultures, describing separately multidrug-resistant (MDR) bacteria. We analyzed the susceptibility to different antibiotics. Results: 51 202 urine cultures were processed during the study; 14 480 (28.3%) cultures were positive. In 11 427 samples Gram negative (79%) were isolated, 2 080 Gram positive (14.4%), and 973 yeasts (6.6%). Escherichia coli was the most frequent bacteria identified (56.1%); 24% of the community strains and 65.7% of the nosocomial were extended-spectrum beta-lactamase producers (ESBL). Klebsiella pneumoniae was isolated in 705 samples (4.8%); 115 were ESBL (16%), 13.1% from community and 29.8% from nosocomial source. Pseudomonas aeruginosa was identified in 593 cultures (4.1%): 9% from community and 51% nosocomial. Conclusions: MDR bacteria were more frequent in nosocomial isolates. It should be a priority to intensify the rational use of antimicrobials in the community and antibiotic stewardship in the hospital.

Resumo Um homem de 60 anos de idade foi submetido a transplante renal em 2013 devido à insuficiência renal crônica causada por hipertensão. Ele teve episódios recorrentes de infecção do trato urinário e veio para o hospital devido a 4 dias de febre, dor abdominal, ardência para urinar e náusea. Análise do sedimento urinário revelou um quadro de infecção (> 50 leucócitos/campo de grande aumento associado à bacteriúria maciça). O sedimento urinário revelou elementos alongados com um alargamento na parte central do corpo da estrutura.


Abstract A 60 year-old man was submitted to kidney transplantation in 2013 due to chronic renal insufficiency caused by hypertension. He had recurrent episodes of urinary tract infection and came to the hospital due to a 4 day-long fever, abdominal pain, burning urination and nausea. Routine urinalysis revealed a picture of infection (> 50 leucocytes/high power field associated to massive bacteriuria). The urine sediment revealed elongated like elements with an enlarged part in the middle of the structure body.

En este estudio fueron analizadas mediante el cultivo muestras de orina de pacientes hospitalizados en la región centro-oeste de Brasil; los microorganismos aislados fueron identificados filogenéticamente como Trichosporon asahii. A través del análisis de máxima parsimonia de las secuencias de IGS1, fueron encontrados 3 genotipos que no habían sido descritos anteriormente. Las concentraciones inhibitorias mínimas frente a los 9 aislados identificados presentaron un rango de 0,06-1µg/ml en el caso de la anfotericina B, de 0,25-4µg/ml en el del fluconazol, y de 0,03-0,06µg/ml en el del itraconazol. Aproximadamente 6/9 de los aislados de T. asahii formaron biopelículas en la superficie de microplacas de poliestireno. Este trabajo documenta el aislamiento de T. asahii como agente causal de infeciones urinarias nosocomiales. Además, demuestra que la región IGS1 puede ser considerada una nueva herramienta epidemiológica para la genotipificación de los aislados de T. asahii. Los genotipos menos comunes encontrados en este estudio pueden estar relacionados con las características epidemiológicas locales


In this study, the culture analysis of urine samples from patients hospitalized in the Central-West region of Brazil was performed, and the isolated microorganisms were phylogenetically identified as Trichosporon asahii. Maximum parsimony analysis of the IGS1 sequences revealed three novel genotypes that have not been described. The minimum inhibitory concentrations of the nine isolates identified were in the range of 0.06­1µg/ml for amphotericin B, 0.25­4µg/ml for fluconazole, and 0.03­0.06µg/ml for itraconazole. Approximately 6/9 of the T. asahii isolates could form biofilms on the surface of polystyrene microplates. This study reports that the microorganisms isolated here as T. asahii are agents of nosocomial urinary tract infections. Furthermore, the IGS1 region can be considered a new epidemiological tool for genotyping T. asahii isolates. The least common genotypes reported in this study can be related to local epidemiological trends

Background: Urinary tract infection is more common among diabetic patients. Aim: To describe the frequency and features of urinary tract infection among diabetic patients admitted to a medicine service in Colombia. Material and Methods: Review of a database of diabetic patients aged over 18 years, admitted to a Medicine Service in Colombia. The medical records and hospital evolution of those with urinary tract infections were reviewed. Results: Data from 470 patients aged 65 +/- 13 years (50 percent females) were reviewed. Urinary tract infections were detected in 68 (14 percent), all community acquired. A culture was done in 50 (73 percent), which was positive in 80 percent. The most common microorganisms isolated were Escherichia coli and Klebsiella pneumoniae in 52 and 18 percent respectively. Twenty eight percent of E Coli strains were resistant to extended spectrum beta lactamases and 14 percent of K pneumoniae strains were resistant to ampicillin. Complications were observed in 28 percent of these patients and 6 percent died, mainly due to septic shock. Glycosylated hemoglobin levels in patients with and without urinary tract infection were 9.2 and 8.5 percent respectively (p = 0.016). Conclusions: Urinary tract infection is common among hospitalized diabetic patients and associated with complications and higher mortality.

ABSTRACT

Enterococcus faecalis (E. faecalis) é um microrganismo presente em lesões endodônticas persistentes, mostrando maior resistência do que outras bactérias ao Hidróxido de Cálcio, um medicamento alcalino que consegue eliminar diversos microrganismos durante o tratamento endodôntico. Assim, os objetivos desse estudo foram: (a) avaliar a resposta de E. faecalis isolados de canal radicular, após estresse alcalino, quanto sobrevivência, crescimento, alteração do pH, resistência/susceptibilidade antimicrobiana e formação de biofilme sobre discos de dentina; (b) avaliar a capacidade fagocítica e produção de óxido nítrico (NO) por macrófagos humanos, frente a bactérias E. faecalis de canais radiculares, submetidas a estresse alcalino; (c) avaliar a expressão de TLR2 e CD14 na superfície dos macrófagos desafiados com as diferentes cepas bacterianas. As cepas utilizadas foram: ATCC4083 (CANAL 1) e uma cepa clínica, obtida por nós, a partir de uma lesão endodôntica primária (CANAL 2), ambas isoladas de canais radiculares; e ATCC29212 isolada de urina (URINA), utilizada como controle. O estresse alcalino foi obtido através da inoculação das bactérias em meio BHIalcalino por 4, 24, 48 e 72 horas. As bactérias alcalino-resistentes foram semeadas em ágar, com ou sem troca do meio, e quantificadas por CFU/mL. A susceptibilidade antimicrobiana das diferentes cepas, estressadas ou não (controle), foi determinada pelo Etest; e o biovolume do biofilme foi quantificado microscopicamente. Para avaliar a capacidade fagocítica, macrófagos obtidos a partir de monócitos do sangue periférico foram desafiados com as diferentes cepas, estressadas ou não em meio BHI-alcalino, por 30 minutos, na proporção 5:1 (bactéria/macrófago), e corados com Laranja de Acridina. Foi contado o total de macrófagos com bactérias internalizadas, considerando o número de bactérias internalizadas por célula (<5 e =5). A concentração de NO foi medida em sobrenadantes, através da reação de Griess, e a expressão de TLR2 e CD14 pelos macrófagos foi analisada por citometria de fluxo. Os resultados revelaram que Enterococcus oriundos de canal radicular foram menos resistentes ao estresse alcalino e mais susceptíveis aos antibióticos testados, do que as bactérias oriundas de urina. A falta de nutrientes foi um fator determinante para o crescimento bacteriano de todas as cepas. O biovolume dos biofilmes foi semelhante para todas as cepas estudadas, e não foi alterado após exposição ao BHI-alcalino. Na presença de bactérias submetidas ao estresse alcalino, houve um menor número de macrófagos com bactérias internalizadas, em comparação ao controle. No entanto, a produção de NO e a expressão de TLR2 e CD14 não foram alteradas. Independentemente da cepa utilizada e da presença de estresse alcalino, a maioria dos macrófagos apresentavam-se com =5 bactérias internalizadas por célula. Na ausência de estresse, as cepas de urina resultaram em maior produção de NO que aquelas oriundas do canal radicular; entretanto, a produção deste gás foi semelhante entre as cepas após estresse alcalino. A partir desses resultados, podemos concluir que bactérias E. faecalis de urina diferem daquelas oriundas do canal radicular, principalmente quanto a susceptibilidade/resistência microbiana; assim sugerimos que estudos envolvendo o campo da Endodontia devam ser realizados com cepas oriundas de canal radicular, preferencialmente que de urina. Concluiu-se ainda que um ambiente alcalino associado a falta de nutrientes pode reduzir o crescimento de E. faecalis. Adicionalmente, o estresse alcalino pode levar a alterações na estrutura da parede de E. faecalis, o que dificulta o seu reconhecimento, reduzindo sua fagocitose, mas não a sua capacidade de ativar a produção de NO, pelos macrófagos. Assim, uma medicação intracanal a base de hidróxido de cálcio associada a restaurações coronais muito bem adaptadas, para se evitar infiltração, é fundamental em tratamentos endodônticos. No entanto, os efeitos do estresse alcalino, nos Enterococcus alcalino-resistentes, podem prejudicar sua fagocitose, contribuindo para sua persistência na doença endodôntica.(AU)


Enterococcus faecalis (E. faecalis) is an microorganism present in persistent endodontic lesions, with greater resistance than other bacteria to the calcium hydroxide, an alkaline intracanal dressing which eliminate several bacterial species during endodontic treatment. The objectives of this study were: (a) to evaluate the response of E. faecalis, isolated from root canal, under alkaline-stress, starvation, antimicrobial resistance/susceptibility and biofilm formation on dentin disks; (b) to evaluate the phagocytic ability and the nitric oxide (NO) concentration of human macrophages against root canal E. faecalis isolates submitted to alkaline stress; (c) to evaluate the intensity of TLR2 and CD14 expression on the surface of macrophages challenged with the different bacterial strains. The bacterial strains used were: ATCC 4083 (CANAL 1) and a clinical strain, obtained by us, from a primary endodontic lesion (CANAL 2), both isolated from pulpless teeth; and ATCC29212, isolated from urine (URINE), was a reference for comparison. All strains were inoculated in alkaline-BHI broth for 4, 24, 48 and 72 hours. The alkalineresistant bacteria were seeded in agar and quantified by CFU/mL. Antimicrobial susceptibility of bacterial strains, stressed or not (control) was determined by the Etest and the biovolume after biofilm formation was quantified by microscopy. To evaluate the phagocytic ability, macrophages obtained by culture of peripheral blood monocyte, were challenged with bacterial strains, stressed or not in BHI-alkaline for 30 minutes at 5:1 ratio (bacteria/macrophages) and stained with Acridine Orange. The total of macrophages with internalized bacteria and also the number of internalized bacteria per cell (<5 and =5) were counted. The NO concentration in the supernatants was measured by Griess reaction and the intensity of TLR2 and CD14 expression on the surface of macrophages was also analyzed by flow cytometry. Results shows less resistance to alkaline stress in root canal strains and less resistance to tested antibiotics when compared with urine enterococci. The lack of nutrient was a determining factor for the bacterial growth in all enterococci strains. The biovolume of biofilm formed by all strains were similar, and were not altered after exposure to an alkaline-BHI. In the presence of alkaline-stressed bacteria, there was a smaller number of macrophages with internalized bacteria, when compared to the control. The NO production or the TLR2 and CD14 expression were not altered. Regardless of the strain or alkaline environment, the number of macrophages that showed =5 internalized bacteria per cell was higher. Without an alkaline-stress the NO production results higher in the urine strain, when compared with the root canal strains, however, was not modificated after the exposure of bacteria to alkalinestress. We conclude that root-canal strains have different features when compared with urine enterococci, with the main differences being evident in their resistance/susceptibility to antibiotics; thus, we suggest that researches with aims directed to interpreting responses to endodontic treatment should be conducted with strains from root-canals. Besides, an alkaline environment associated to a starvation condition can reduce bacterial growth. Additionally, alterations in the structure of bacterial cell wall after alkali-stressing possibly made their recognition difficult, reducing their ability to be phagocytized, but not their ability to activate NO production. Therefore, intracanal medication with calcium hidroxyde dressing and coronal restorations, to prevent infiltration, should be critical in treatments of endodontics infections. However, the impact of alkaline stress, in alkaline-resistant enterococci, can impair the phagocytosis, contributing to their persistence in endodontic disease.(AU)

ABSTRACT

BACKGROUND/AIMS: Efforts to decrease the use of extended-spectrum cephalosporins are required to prevent the selection and transmission of multi-drug resistant pathogens, such as extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae. The objectives of this study were to assess the clinical efficacy of intravenous cefuroxime as an empirical antibiotic for the treatment of hospitalized women with acute pyelonephritis (APN) caused by Escherichia coli. METHODS: We analyzed the clinical and microbiologic database of 328 hospitalized women with community-onset APN. RESULTS: Of 328 women with APN, 22 patients had cefuroxime-resistant E. coli APN, and 306 patients had cefuroxime-susceptible E. coli APN. The early clinical success rates were significantly higher (p = 0.001) in the cefuroxime-susceptible group (90.8%, 278/306) than in the cefuroxime-resistant group (68.2%, 15/22) at 72 hours. The clinical cure rates at 4 to 14 days after completing antimicrobial therapy were not significantly different in the cefuroxime-resistant or -susceptible groups, with 88.2% (15/17) and 97.8% (223/228; p = 0.078), respectively. The microbiological cure rates were not significantly different and were 90.9% (10/11) and 93.4% (128/137), respectively (p =0.550). The median duration of hospitalization in the cefuroxime-resistant and -susceptible groups was 10 days (interquartile range [IQR], 8 to 13) and 10 days (IQR, 8 to 14), respectively (p =0.319). CONCLUSIONS: Cefuroxime, a second-generation cephalosporin, can be used for the initial empirical therapy of community-onset APN if tailored according to uropathogen identification and susceptibility results, especially in areas where the prevalence rate of ESBL-producing uropathogens is low.

ABSTRACT

BACKGROUND/AIMS: The number of urinary tract infections (UTIs) caused by extended-spectrum beta-lactamase-producing Escherichia coli (ESBL-EC) is increasing. In an outpatient setting, there are limited therapeutic options to treat ESBL-producing pathogens. We evaluated the outcomes of amikacin outpatient parenteral antibiotic therapy (OPAT) for UTIs caused by ESBL-EC in patients not pre-treated with carbapenem. METHODS: We retrospectively evaluated the outcomes of amikacin OPAT for UTIs caused by ESBL-EC. RESULTS: From November 2011 to October 2012, eight females, who could not be hospitalized for carbapenem treatment, were treated with amikacin OPAT for nine episodes of non-bacteremic ESBL-EC UTIs. Seven of the eight patients had one or more comorbidities. Of the nine UTI cases, three had symptomatic lower UTIs and six had non-bacteremic upper UTIs. In all of the cases, symptomatic and laboratory improvements were observed following amikacin OPAT. One patient showed a delayed relapse with bilateral microabscesses 3 weeks after treatment cessation; however, a clinical and microbiological cure was eventually reached. All of the patients were able to tolerate amikacin OPAT without any significant nephrotoxicity or ototoxicity. CONCLUSIONS: Amikacin OPAT represents a feasible therapeutic option for non-bacteremic UTIs caused by ESBL-EC in settings with limited resources.

This study investigated the occurrence of canine distemper virus (CDV) by evaluating the presence of viral RNA within urine samples of dogs from Uberlândia, MG, with clinical manifestations suggestive of infection by CDV by targeting the CDV N gene. Of the clinical samples collected ( n =33), CDV viruria was detected in 45.5%. Five dogs died spontaneously; all had characteristic CDV-associated histopathological alterations and demonstrated CDV viruria. Statistical analyses revealed that the age, gender, breed, or the organ system of the dog affected had no influence on the occurrence of canine distemper. Myoclonus and motor incoordination were the most significant neurological manifestations observed. A direct association was observed between keratoconjunctivitis and dogs with CDV viruria. These findings suggest that CDV viruria in symptomatic dogs might not be age related, and that symptomatic dogs can demonstrate clinical manifestations attributed to CDV without viruria identified by RT-PCR. Additionally, the results of the sequence identities analysed have suggested that all Brazilian wild-type strains of CDV currently identified are closely related and probably originated from the same lineage of CDV. Nevertheless, phylogenetic analyses suggest that there are different clusters of wild-type strains of CDV circulating within urban canine populations in Brazil.


A presença do ácido nucleico (RNA) do vírus da cinomose canina (CDV) foi avaliada por meio da amplificação parcial do gene N pela técnica RT-PCR realizada em urina de cães provenientes de Uberlândia, Minas Gerais, que apresentavam sinais clínicos sugestivos de cinomose. Das 33 amostras de urina avaliadas, o CDV foi identificado em 45,5%. Em cinco cães que morreram espontaneamente, além da excreção do CDV na urina, foram observadas alterações histopatológicas associadas à infecção por esse vírus. Análises estatísticas demonstraram que a idade, gênero, raça e o sistema orgânico comprometido dos cães avaliados não exerceram influência no diagnóstico da cinomose canina. Mioclonia e incoordenação motora foram as manifestações neurológicas que apresentaram frequência de ocorrência significativa (P<0,05). Uma associação direta foi observada entre a presença de ceratoconjuntivite e a identificação de virúria pelo CDV. Esses achados sugerem que a excreção do CDV pela urina em cães com sinais clínicos compatíveis com cinomose pode não ser relacionada com a idade do animal, e que animais sintomáticos podem apresentar manifestações clínicas atribuídas ao CDV, porém sem a caracterização de virúria por RT-PCR. Adicionalmente, análises filogenéticas sugerem que várias cepas de CDV podem estar circulando em populações caninas de áreas urbanas no Brasil.

ABSTRACT

A epididimite infecciosa ovina é uma das principais enfermidades reprodutivas de carneiros. O presente estudo teve por objetivo desenvolver protocolos de PCR em tempo real para B. ovis e H. somni e avaliar sua aplicabilidade em amostras de sêmen e urina de carneiros. Delinearam-se primers e sondas espécie-específicos para cada agente. As sondas foram delineadas com o sistema TaqMan incorporando um marcador FAM para B. ovis e Cy5 para H. somni na extremidade 5' e um quencher na extremidade 3'. A PCR em tempo real para B. ovis e H. somni foi altamente sensível, uma vez que a amplificação de DNA ocorreu com até 0,2ng de DNA/reação. A especificidade dos iniciadores e sondas foi avaliada com amostras de DNA de outros agentes causadores de epididimite ovina e nenhuma amplificação inespecífica foi observada. A aplicabilidade da técnica em amostras biológicas também foi confirmada, pois não houve perda de eficácia (P>0,05) quando comparada à PCR convencional com amostras de sêmen e urina de carneiros experimentalmente infectados.

Introducción. Las infecciones por Chlamydia trachomatis y Neisseria gonorrhoeae son las dos infecciones bacterianas de transmisión sexual más frecuentes en el mundo, principalmente en menores de 25 años, en cuya salud sexual, reproductiva y materno-infantil producen secuelas. Objetivo. Estimar la prevalencia de las infecciones por C. trachomatis y N. gonorrhoeae entre adolescentes en colegios de la provincia de Sabana Centro, Cundinamarca, Colombia. Materiales y métodos. Se hizo un estudio de corte transversal en adolescentes de 14 a 19 años matriculados en colegios ubicados en los 11 municipios de la zona de estudio. Después de obtener el consentimiento informado, los participantes llenaron una encuesta y recolectaron una muestra de orina para la detección de C. trachomatis y N. gonorrhoeae mediante la técnica de reacción en cadena de la polimerasa en tiempo real. Resultados. En una muestra de 972 adolescentes sexualmente activos, se encontró una prevalencia de infección por C. trachomatis del 2,24 % y, por N. gonorrhoeae , del 0,10 %. No se detectaron infecciones concomitantes. De las estudiantes positivas para C. trachomatis , el 21,4 % eran asintomáticas. Se encontró una relación significativa entre la infección por C. trachomatis y el sexo femenino, el consumo de alcohol u otras sustancias durante la última relación sexual, el tener una o varias parejas y la presencia de alguna infección de transmisión sexual. Conclusiones. La prevalencia de infecciones por C. trachomatis fue inferior a la reportada en grupos similares y se presentaron menos casos asintomáticos en comparación con la tendencia general. Se recomienda implementar en el país un sistema de vigilancia epidemiológica para estas dos infecciones de transmisión sexual mediante la tamización con técnicas moleculares no invasivas, para priorizar estrategias preventivas en las poblaciones de mayor riesgo.

These infections affect principally less than 25-year-old individuals with sexual, reproductive and maternal and child health consequences. Objective: This study sought to estimate the prevalence of C. trachomatis and N. gonorrhoeae among high school students in the 11 municipalities of the Sabana Central area of Cundinamarca, Colombia. Materials and methods: A cross-sectional study was conducted among 14 to 19-year old adolescents attending schools in the study area. After signing their consent, participants completed a questionnaire and provided a urine sample for the detection of C. trachomatis and N. gonorrhoeae by real-time polymerase chain reaction. Results: In the sample of 972 sexually active adolescents, the prevalence of C. trachomatis and N. gonorrhoeae was 2.24% and 0.10%, respectively. No co-infections were detected. Overall, 21.4% of women who tested positive for C. trachomatis were asymptomatic. Significant relationships were found between C. trachomatis infection and female gender, the consumption of alcohol or other substances during the previous sexual relationship, having one or more partners and the presence of sexually transmitted infections. Conclusions: The prevalence of C. trachomatis detected in this study was lower than the prevalence reported for similar groups; besides, contrary to the general trends, fewer asymptomatic cases were identified in this study. To prioritize preventive strategies in high-risk populations, an epidemiological surveillance system for C. trachomatis and N. gonorrhoeae should be implemented in Colombia, with screening performed using non-invasive molecular techniques.
